Included Health is the first comprehensive health platform for the LGBTQ+ community. We are
seeking a Care Coordinator to join our team as we launch with new clients, expanding our
member base. Included Health Care Coordinators serve as the primary contact for our members
and are responsible for ensuring each one is able to be connected to appropriate resources.
This person will correspond with members via email, text and phone to understand the
member’s needs and ensure we are meeting them. They will be responsible for outreach to
doctor offices to ensure they meet our Included Health LGBTQ+ friendly criteria and confirm
they are a good fit for our members. We strive to provide a concierge experience for all of our
members and the Care Coordinator is at the heart of that for our members.
Who you are:
You are passionate about advocating for the LGBTQ+ community. You understand the pain,
frustration, and/or embarrassment associated with navigating the healthcare system as an
LGBTQ+ individual. You are a self-starter who likes to take ownership of your work and you
proactively go the extra mile for others. You’ll ensure you are communicating with your
members in a timely manner and following up on their requests diligently. You like a challenge
and are eager to collaborate with your fellow team members and research to find solutions.
Attention to detail is one of your strengths. You are comfortable with both written and verbal
communication to many groups of people (members, resource groups, doctor’s offices, etc.).

● Excellent interpersonal and organizational skills
● Experience handling outbound and inbound calls
● Strong written and verbal communication skills
● Basic understanding of healthcare and navigating insurance and doctor visits
● Flexible and adaptable to new systems and tools
● Basic proficiency with Google sheets and internet research
● Associates or Bachelors degree
● Experience working in a customer facing role
