Benefits leaders have been addressing diabetes and weight management concerns for years. But, the recent attention on glucagon-like peptide-1 (GLP-1) medications is adding new dynamics to the equation. Although these medications have been around for years, clinical and societal dynamics have created an onslaught of demand for these expensive drugs, leaving employers, health plans, and providers scrambling to figure out their strategy for providing appropriate access to GLP-1 medications while also controlling costs.

Does your company employ expats or employees that travel internationally for business? Special health situations can arise for these workers, such as:

  • Medical situations that occur while outside of an employee’s home country may require support to ensure that the employee is receiving appropriate care at an appropriate facility, sometimes even including medical evacuation to another facility or country

  • If exposure to infectious diseases (e.g., tuberculosis, COVID, measles) occurs while out of the country (or after return) it can also impact coworker safety and have public health implications

  • In addition, required immunizations and other regulatory and health & safety requirements must be met as a part of international travel policy

 

Many employers do not have the expertise available to navigate these situations, which can sometimes be time-critical and life-threatening

Case Study Examples

  1. Infectious disease exposure An employer became aware of an active TB case discovered in a foreign employee visiting the US. There were urgent concerns regarding business operations and potential exposure. Our CMO onDemand expert assisted the employer in:

    • Understanding the issues and implications from clinical, business, health & safety and regulatory perspectives

    • Identifying additional exposures through contact tracing, including others who might need to be notified and/or quarantined

    • Interfacing with state and local public health authorities

    • Communicating to impacted employee groups with authoritative health information and guidance, answering questions in real time
       

  2. Medical Evacuation An employee traveling in SE Asia had an episode of chest pain. The local hospital determined it was a heart attack, and recommended a procedure to open the obstructed coronary artery. There were concerns about the quality of care available locally. Our CMO onDemand expert assisted the employer in:

    • Identifying a location where the quality of care was sufficient to safely perform the procedure

    • Advising the employer and family members of the situation and the recommendations for evacuation

    • Working with a medical evacuation vendor to arrange transport by air ambulance

    • Reviewing plans and progress before, during, and after the medical evacuation and the procedure

Chief Medical Officers onDemand has recently added a new advisor to our expert panel, Paul Hodgins, MD, MPH. Paul has over 25 years of experience in developing and implementing policies, programs and services to address the health-related needs of employees and businesses. He recently retired as the Chief Medical Officer for ConocoPhillips, and previously served in leadership roles in General Electric and Caterpillar. His experience includes leading global health teams, implementing wellness programs, onsite clinics, ensuring regulatory compliance, and assessing and advising businesses on a broad range of local and global health-related issues.

 

Special interests include physical and mental wellbeing, reducing personal and occupational health risks, healthcare cost containment, improving access to healthcare through onsite clinics and leveraging local hospital relationships, and tailoring policies, programs and services to meet employee and business needs that vary widely by location.

 

Dr. Hodgins is a graduate of Trinity College Dublin, Ireland, and he completed residency training in Occupational and Environmental Medicine, and a Master of Public Health degree at Emory University.  He completed a residency in Internal Medicine at the University of Connecticut. He also served as a Medical Officer with the Centers for Disease Control and Prevention.

Sometimes when a plan member is denied coverage for a treatment that they and their physician believe is appropriate, they appeal the carrier’s decision. How do you, as an employer, ensure that your members’ concerns are thoroughly evaluated and supported, while still supporting the objectives of the health plan?  

 

Our CMO onDemand experts can interface with your carrier to help resolve the appeal.  We will provide an independent medical review of records, drawing on our deep medical expertise to research and review clinical appropriateness and cost-effectiveness of treatments, tests, and other services. This requires a clinical understanding of standards of practice and ability to research alternative services, recommendations and endorsements by medical societies, governmental approvals, and benchmarking against the marketplace.

 

Sometimes an appeal review will also highlight opportunities to modify your plan design provisions to accommodate similar future clinical situations.

 

Case Study Example

  • An employee’s spouse with Glioblastoma Multiforme was denied treatment with Optune (electric tumor treatment field therapy).

  • Our medical expert discovered that denial was based on medical necessity guidelines from a two-year old document. Under recent changes in FDA guidance and NCCN Guidelines, this patient’s tumor met the guidelines for the relatively new therapy.

  • The denial was overturned and treatment was initiated.

Did you know that less than 3% of your members likely drive over 50% of your medical and Rx costs? How can you monitor emerging complex high-cost cases in a timely manner, and stay on top of your carriers, TPAs, medical supply, condition management, and other vendors that heavily impact these claimants? 

 

Our CMO onDemand team can help you remain vigilant, reviewing high-cost medical claimants quarterly and auditing relevant provider charges. We can also review your strategies and interventions for critical areas such as cancer, back surgery, neonatal care, and conditions treated with specialty medicine. In many cases, review of medical records can reveal opportune interventions.

 

For many high cost claimants, more targeted or specialized solutions are available (e.g., air transport, supplier of specialty medicines, carve-out third party solutions for cancer, neonatal and back surgery, and expert medical opinion services to validate treatment plans). These solutions are sometimes identified by intermediaries or stop loss carriers. Our expert team can help you sort through your options, with an independent perspective, to help you choose the best path.

 

Here are a few examples of how we’ve helped in this area:

  • A review of high-cost claimants revealed 3 individuals with hemophilia (all from the same family). The CMO OnDemand expert suggested contacting the stop loss carrier who identified a third-party vendor partner who could significantly reduce (45% reduction in costs) the high costs of therapy for these patients.

  • A claimant was receiving a treatment which the TPA had allowed, but which the CMO OnDemand expert considered unproven. The TPA was notified and subsequently intervened with the treating provider. The claimant switched providers and received a more traditional, less costly treatment.

  • A claimant was transferred from an intensive care unit directly to a skilled nursing facility. The CMO OnDemand expert suggested that ICU care had been extended inappropriately, and that the claimant only required intensive care for 29 of the 67 days it was received. This led to renegotiation of the hospital bill with a reduction of $78,000.

10 Things Employers Should Know About The New OSHA COVID-19 Guidance

….plus some proactive considerations from the CMO onDemand Panel

 

On June 10, 2021, OSHA released its highly-anticipated guidance for employers related to COVID-19 worker safety. This included updated guidelines for all employers, as well as an Emergency Temporary Standard (which became effective on June 21, 2021) applicable only to the healthcare industry.  Below are some highlights for employers (focusing on the guidelines
for non-healthcare settings) as well as additional insights and considerations from CMO
onDemand panelists.

 

  1. Vaccination remains the “key” in a “multi-layered approach to protect workers”. Employers should grant paid time off for vaccinations and may wish to further incentivize vaccination through other policies and programs.
     

  2. Symptomatic workers should continue to stay home as well as unvaccinated or infected workers who have had confirmed close contact with someone who has tested positive for COVID-19. Employers should also build non-punitive absence policies. Continue to build a health culture around prevention: when in doubt, stay home.
     

  3. Provide unvaccinated and high-risk individuals with face coverings unless their activities require more extensive forms of PPE and maintain physical distancing in all communal work areas. In addition, encourage unvaccinated customers and visitors to wear face coverings. As we return to a focus on individual risk in our prevention at work, the choice of masks (e.g. cloth vs. KN95) also matters significantly.
     

  4. Suggest that unvaccinated customers, visitors or guests wear face coverings. There are continued risks for those employees who are unvaccinated or at high-risk and have returned to onsite work. The most common approach is self-attestation and posted communications indicating the presence of unvaccinated workers and/or high-risk individuals. 
     

  5. On-site investments in ventilation and routine cleaning and disinfection should continue. Diligent disinfection protocols should continue, particularly if someone is suspected/confirmed to have COVID-19. Ventilation systems have been shown to have tremendous impact in preventing disease transmission. Some employers may wish to continue to upgrade these important systems in the COVID era and beyond.
     

  6. Record and report COVID-19 infections and deaths to the appropriate regulatory organizations for work-related infection. Consider the implementation of more extensive infection tracking and disease prevention metrics.
     

  7. Implement protections from retaliation and build processes for workers to communicate concerns and offer feedback. As has been shown in return-to-work planning, collecting trust-based and anonymous sentiments to address employee concerns is imperative in architecting the future of work.
     

  8. There are special considerations for workplaces where contact among mixed-vaccination status workers is close, long in duration or contact is possible through shared spaces. These considerations are for situations such as processing or assembly lines, in retail workplaces, those with communal housing and employer-provided transportation. EHIR’s bi-weekly COVID collaboration calls among Members can provide turnkey learnings and insights from other employers on practical approaches to mixed immunity populations.
     

  9. OSHA guidelines do not address those workers with high-risk individuals in their families or in the home setting as well as those workers living in proximity to individuals who cannot be vaccinated. It is still possible for a vaccinated individual to become infected and transmit COVID-19 to a family member. To that end, employers should carefully consider requests from such workers for continued on-site precautions and continued accommodation of remote working where available.

Vaccine Hesitancy vs. Vaccine Confidence

An emerging conversation among employers is how to help employees overcome “vaccine hesitancy.” For a host of different reasons, concerns, and fears, a large percentage of our population continues to be uncomfortable with obtaining a vaccination. In November 2020, 21% of U.S. adults indicated that they don’t plan to get vaccinated, even if more information becomes available (Pew Research Center).

 

There is much discussion among employers on what role they might take in encouraging vaccination for all workers who could benefit from it. Having a larger percentage of employees vaccinated offers significant benefits:

  • Helping ensure longer-term workforce health and performance 

  • Reducing anxiety and/or fear of contagion in the workplace

  • Protecting against COVID-19 outbreaks within a plant or other business location

  • Reducing the number of lost employee days due to quarantining since in most cases, future quarantining will be required only for unvaccinated individuals who have been exposed to someone with COVID-19

 

With so much pervasive misinformation, it can be tempting to launch a behavior change battle on vaccine hesitancy, even dismissing those who have questions or are hesitant. But research shows that positive emotional messages, such as altruism and hope, are more effective than negative ones at engaging people and inspiring action (Chou, W. S., & Budenz, A., Health Communication, Vol. 35, No. 14, 2020). Rather than telling people why their hesitancy is unfounded, work to compassionately educate and build their knowledge and understanding of the rapidly emerging evidence of vaccine safety and effectiveness.  Psychological studies on vaccine hesitancy indicate that changing people’s thoughts and feelings is very difficult, but changing social processes and leveraging positive intentions and trusted role models can be effective ways to stimulate action (Brewer, N. T., et al. Psychological Science in the Public Interest, 2017).

As a result, we’ve come up with the following “Top Ten” questions you should be asking yourself about your organization’s readiness to address the needs of your population when it comes to COVID vaccines.

 

Top Ten Questions for Employers - COVID Vaccine Strategy

  1. Have we identified a person and/or committee who are accountable and leading our efforts on vaccine strategy? Does the committee have project management support?

  2. Do we have a firm understanding of our population’s likelihood to take a vaccine? 

  3. Which subgroups within our population are likely to qualify first for a vaccine?  Do we have an essential workforce?  Do we have an elderly workforce?

  4. Have we aligned on a philosophical approach to vaccine deployment which will guide decisions around vaccine promotion, mandates, incentives?  

  5. Have we implemented the policies and procedures to back our philosophical approach (e.g., PTO for vaccination time/recovery, inclusion of dependents, etc.)? 

  6. Would we provide vaccinations to family members of our employees, contractors and other non-benefits-eligible employees?

  7. Can or should we be involved in the procurement or acquisition of vaccine through state and local government, or having vendors do so on our behalf? 

  8. Do we have a well-formulated policy and corporate relations perspective on health equity concerns associated with securing vaccine for our population? 

  9. Have we considered the full range of options available for off-site, and on-site administration of the vaccine including vendors, pharmacies, health plans and providers?  

  10. Is our organization prepared to act on the implications of vaccine adherence or non-adherence for issues such as return to work, return to travel?

Barry Hall has joined EHIR as the new Executive Director of CMO onDemand. You may know Barry from his active participation in EHIR Cohort 1 representing Comcast.

Barry has 35 years’ experience in health and benefits strategy and innovation. Most recently, he was the Executive Director of the Office of the Chief Medical Officer for Comcast NBCUniversal, responsible for onsite, nearsite and virtual primary care clinics, enterprise-wide medical policies and guidelines, and internal COVID-19 pandemic response efforts. He was also instrumental in creating the Total Rewards Innovation and Product group at Comcast, where he led innovation strategy, vendor research and evaluation, pilot design and execution, business case development, and prioritization of strategic opportunities across all global benefits areas. 

Barry has an extensive background as a partner/principal for leading consulting firms including PricewaterhouseCoopers, Buck Consultants, and Chicago Consulting Actuaries, where he worked with employers to maximize the health and productivity of their global workforce.

Barry is a Fellow of the Society of Actuaries and a recognized thought leader in health innovation. US employers with essential workforces are reeling amidst a new set of regulatory and public health guidelines in states like New Mexico and California which require providing COVID-19 testing to employees at no cost. Further, some guidelines are allowing public health authorities to close retailers, grocers, restaurants and other businesses with as few as two positive cases in a workplace. In order to re-open, these businesses are being forced to adopt new protocols and build new systems as well as conduct screening tests for the entire workplace as frequently as twice a week. 

 

CMO OnDemand has been fielding requests from EHIR members immediately impacted to assist in identifying testing vendors. Here are a few guidelines to keep in mind when selecting services:

 

Testing Platforms: Molecular PCR testing continues to lead the way in terms of the most practical testing methodology. Rapid diagnostics, although promising lower cost, are still not widely available enough to meet the needs of large employers and they are, by-nature, low-throughput. A handful of vendors are now available and offering rapid diagnostics – should you need to access the list, let us know. 

 

Demand / Capacity Mismatch: Demand for testing is increasingly rapidly and testing capacity is being stretched significantly. In some cases, it has been hard to reach the vendors and secure initial proposals given the sheer volume of requests. As has been the case throughout the pandemic, demand appears to be outpacing capacity – if additional states implement similar standards, securing these testing services could become impractical given the frequency of testing required

 

Flexibility: Employers who can be flexible in testing platforms and sample collection methodologies are likely to find the vendors they need. For instance, most employers we have engaged in the last two weeks are aiming for full-service testing which includes on-site teams of swabbers, full logistics of sample delivery to laboratories and automated reporting of results. 

 

Self-Sample Collection: Those who can utilize at-home testing programs with self-sample collection application may find more options available to them to meet the new stringent requirements – although the costs here can be high. An alternative to lowering these costs is to engage a laboratory who can ship sample collection supplies to individual workplaces and allow employees to conduct self-collection followed by shipment of large batches to individual laboratories. 

 

On-site Clinical Services: If employers have access to on-site or near-site clinical services, they may also choose to procure their own sample collection supplies and simply engage reference laboratories to conduct the molecular testing. 


Results Reporting: Self-reporting test results through a daily symptom checker/exposure control application remains a viable approach to avoiding the need for complex reporting systems if your organization has not yet established such systems. Some testing vendors are providing turn-key results reporting, but not all offer such sophisticated systems.

Here are four ways we’ve been bringing clinical insights to our clients: 

 

COVID-19, all-the-time: CMO onDemand’s pandemic response begins with helping employers sort out the art vs. science <insert link to last month’s article> associated with rebuilding business processes amidst a constantly evolving regulatory environment. As the pandemic unfolds, we’re increasingly assisting proactive employers with surveillance testing protocols and determining the role of lab-based PCR testing vis-à-vis an increasing array of less expensive, rapid diagnostic choices. In addition, as businesses test return to normal on an incremental basis, we’re there to provide the clinical insights to determine which core business processes to reintroduce as-is, which to modify and which to redesign from the ground up. 

 

Mental Health: Often referred to as the ‘silent’ pandemic, COVID-19 has ushered in an era of increased anxiety, elevated rates of depression, familial tensions and resulting decreased productivity. One way we are tackling mental health concerns has been to partner with Dr. Jennifer Riff to launch a series of workshops for EHIR members. Our goal: to get the conversation started within major employers and give employees space and permission to recognize that on some days, ‘I’m just not okay’. Together with Dr. Riff we are working across management teams to provide toolkits for building resiliency, dealing with uncertainty, coping with stress and ways to build a more proactive culture of positive mental health. 

 

Strategic population health management: Despite signs that the COVID-19 pandemic may not be waning anytime soon, many EHIR members are getting back to driving their strategic agenda. As such, CMO OnDemand is increasingly re-engaging in advising around strategic health programs including cancer, musculoskeletal, executive health and on-site/near-site clinic management. 

 

Population Health Analytics: Benefits leaders often decry their lack of control around a strategic agenda given the intricacies of working across multiple carriers and numerous point solutions – all of which end up driving a coordinated population health strategy. One way we see EHIR members taking new steps to drive a more concerted strategy in population health is being more prescriptive with those carriers and vendors about the outcomes they wish to achieve. Together with a handful of design and analytics experts, CMOD is beginning work this Autumn to bring new, clinical insights to the development of population health analytics which enable more control over measuring and monitoring key population health outcomes across an increasingly complex array of vendors, carriers, consultants and other stakeholders.

COVID-19 employer conundrum - Sound Science vs. Art in clinical decision support: Clinicians have long prided themselves in making population health decisions based on sound science, in the form of peer-reviewed literature and guidance from a handful of regulatory and public health institutes. An unfortunate by-product of this pandemic has seen the regulatory environment around COVID-19 get more and more murky. In any given week most of us feel as if HHS, FDA, CDC, WHO, the White House and individual state/municipal regulatory agencies are at odds with one another – sometimes issuing contradicting guidance across similar issues in parallel, only to be changed days later depending on the interpretation and incorporation of guidance from a ‘sister’ regulatory agency or institute. For public health professionals and employers alike, trying to keep up on the constant change is nearly impossible. And the peer-review process just wasn’t set up to be operational amidst a fast-moving pandemic. As clinical experts, we have often found ourselves balancing sound science with some periodic ‘art’ thrown in to help guide practical decisions which employers can react to and build businesses around. 

 

Regulatory (mis) guidance: Just this past week, the CDC issued guidance against any asymptomatic COVID-19 testing, only to walk back those recommendations merely days later casting decision making into a whirlwind for public health officials and employers alike  Last month, the CDC adopted the practical approach already taken by many employers, to leverage a more lenient symptoms-based return-to-work strategy amidst lack of access to testing. Previously, an impractical, dual-negative test framework had been suggested. Even amidst the changes, it took nearly a month for some public health institutes and even the CDC itself, to align other guidance within certain vulnerable populations such as employers, health care workers or long-term care facilities, making it difficult for business leaders to interpret and pivot decision-making. 

Proactive Testing: If you are among those employers looking to be proactive in periodic surveillance testing of employees to better identify pre or asymptomatic workers before they have a chance to infect others in the workplace, where would you turn for guidance on a testing protocol? Unless you happen to run a long-term care or assisted living facility, you won’t find any US regulatory body or public health institute providing guidance around building a proactive testing protocol.  Those regulatory bodies argue today that the science just isn’t available….yet. But is such a novel approach so risky amidst the scientific uncertainty that we shouldn’t try in the right situations? 

 

Turn-around-time trumps accuracy: Throughout this pandemic PCR tests have been held up as the go-to, gold standard for COVID testing due to their accuracy (high sensitivity and specificity). And many platforms spanning a variety of different specimen collection mechanisms (nasal swabs, saliva, et al) have received Emergency Use Authorization from a regulatory standpoint. But what if you can’t get test results for 5 days or even 14? At what point is the ‘gold standard’ PCR test no longer actionable from a clinical or even return-to-work context? A handful of experts including Michael Mina at the Harvard T.H. Chan School of Public health as well those led by Raj Shah at Rockefeller Foundation are arguing for a new strategy – one focused on Rapid Antigen testing which delivers results in 15 minutes. Imagine you are the CEO of a urban tour operator once the pandemic begins to wane in the months to come and you are faced with an infection exposure incidence your staff. Should you rely on self-administered PCR tests which take days to return results? Or can you invest now in inexpensive rapid antigen tests which will enable decisions as to who goes to quarantine and who continues at work in 15 minutes. These experts aren’t setting aside science in favor of art – they are just flexing their instincts around artful interpretation and what tools are most practical to protect health and keep businesses running. 

 

We believe that leadership from businesses, armed with key insights from leading scientists, will lead us out of this pandemic.  Amidst the steady stream of changing guidance and expertise in a fluid situation, we find ourselves incorporating more ‘art’ than ever before – but at its core, the science must be sound. Dr. Tom Farris

Most recently the Corporate Medical Director for Kimberly-Clark Corporation, Dr. Fariss now provides a wide-range of counsel to EHIR members including health risk assessment and communication, occupational health compliance, pandemic response strategic onsite clinic management and health program development in cancer, behavioral health, musculoskeletal and beyond.

Dr. Tom Fariss is a founding member of the CMO OnDemand panel.  He has over 25 years’ experience leading global preventive medicine and occupational health and safety programs and recently retired as the VP, Corporate Medical Director for Kimberly-Clark Corporation. Dr. Fariss continues to serve multiple clients through CMO onDemand on topics including occupational and environmental medicine policies and programs, global health and safety, preventive medicine, clinical quality and governance, medical malpractice review and most recently, pandemic response.   

Dr. Fariss is a Founding Board member of the Health Enhancement Research Organization (HERO), holds a current position as Adjunct Professor of Environmental Health in the Emory University School of Public Health and previous clinical positions with the Central Intelligence Agency and National Hospital for Orthopedics and Rehabilitation. His specific areas of interest include health risk assessment and communication, occupational health compliance, medico-legal aspects of employee health, medical surveillance, cancer cluster evaluation and causation, onsite medical clinics, travel medicine, infectious diseases and pandemic planning in the workplace, employee assistance programs and behavioral health, and preventive medicine.

We sat down to learn more about Dr. Fariss and his contributions as a corporate medical director. 

Tell us about your background and how you first became interested in occupational and environmental medicine.

I’m originally from Virginia, attended the College of William and Mary for undergraduate studies and University of Virginia School of Medicine, and I completed my internal medicine residency at Georgetown University. I knew I wanted to subspecialize but hadn’t decided which field at that time. I was leaning toward cardiology, but my wife Margaret introduced me to occupational and environmental medicine at the CIA headquarters campus in Langley, VA, where she was working as a nurse and where I started working part-time. Coincidentally George Washington University School of Public Health started a new fellowship in occupational and environmental medicine later that year, and I became their first fellow. The combination of clinical medicine, prevention, occupational and environmental health, risk assessment, health policy, epidemiology and biostatistics were all fascinating to me and I knew this was the ideal career choice for me.

 

What do you see as your particular interests and areas of expertise as a chief medical officer?

I have continued to practice clinical medicine and see patients while I took on more administrative and leadership responsibilities through my career. Areas that I have a particular interest in include preventive health, health risk assessment, leadership and communication, clinical quality, and medico-legal aspects of occupational health and medical care. I am also a competitive tennis player and have had a strong interest in the science of health and human performance, both mentally and physically.

 

What is an accomplishment that you are proud of and would like to highlight from your career?

I am proud of the evidence-based materials and training I have developed on prevention, healthy lifestyles, fitness, nutrition, sleep, as well as brain health and resilience.

 

How have you found the transition from a full-time medical director in a global organization to being focused on serving multiple organizations in your role at CMO OnDemand?

Working as a full-time medical director for most of my career has been excellent preparation for my role with multiple organizations through CMO OnDemand. Still, I’m learning every day and enjoying the interactions with new teams from very different companies and organizational cultures. It’s great to be able to work with companies on focused problems that may not require a full-time medical director but that can significantly impact the health and safety of employees and family members in their organization.

What impact do you anticipate the COVID-19 pandemic will have on employee health programs, medical benefits, and population health management as we move forward?

It’s already had a profound impact of course, but the pandemic will push us to re-evaluate everything about our employee health programs in the future. Key areas of improvement will certainly include better preparation for similar events and protecting vulnerable populations better. An area of particular interest for me will be the importance of preventive health and healthy lifestyles, which were highlighted as gaps by this pandemic. Every organization will likely be asking questions such as what went well, what could have gone better, and what do we need to put into place moving forward. I believe the panel at CMO OnDemand will be able to help with those important discussions and I’m excited to be a part of it.
